The BWC requires a form called “First Report of Injury” or FROI. This can be filed by an injured worker, the attending physician or health care provider, or the employer. Hospitals and doctor’s offices have this form. It is also available from the BWC website at http://www.bwc.ohio.gov and can be completed online by the injured worker. A claim number is immediately assigned and this can be used for further medical treatment. The sooner this is filed, the quicker the BWC can start processing the claim.
An official claim number can only be issued by the State of Ohio Bureau of Workers’ Compensation. Employers all have service companies or third party administrators that represent their interests. You may get a letter from such a company saying they are assisting the employer in processing the claim and there is an internal number on the letter. This is not an official BWC claim number.
